What is a good program that generates custom passwords
for each site from a single master password?

(And yes, I know that most such programs are vulnerable
to the master password being cracked by dictionary
attack, allowing a bait site to log in to more valuable
sites, but my high value passwords are not vulnerable to
dictionary attacks.)

Someone was proposing to write such a program integrated
with Firefox.  Does such a Firefox addon exist?
